Making an Appointment

To Make an In-Office Appointment:

Please call our office at 408-294-7551 (our preferred choice of communication), staff@sivalleyderm.com,

To schedule a telemedicine visit, please call our office for instructions.

Once a telemedicine visit has been made, our system will send you an automated e-mail with a log-in link that will expire within 24 hours. Our telemedicine portal is through PocketPatient app. Once you receive the link, use it to log in and set up your account with PocketPatient within 24 hours.

Telemedicine is best if you use a quality camera such as that on your  iPhone or Samsung Galaxy Android. It is easier to move a phone around to show the various parts of the body than showing it to a webcam on your computer.
